Description

We are seeking a Certified Health Coach who is bilingual in Spanish and English, with native or near-native Spanish fluency, to support participants enrolled in a 6-month lifestyle medicine series using Whole Health Partners’ curriculum. This role focuses on relationship-based coaching, cultural responsiveness, and continuity of care in collaboration with nurse practitioners and other providers. 
Key Responsibilities 
Coach alongside a five-session live lifestyle medicine class series (Spanish and/or bilingual) 
Provide ongoing participant support through secure text messaging and virtual coaching calls 
Communicate with nurse practitioners and other providers to support continuity of care 
Deliver coaching that is culturally responsive and linguistically appropriate 
Maintain documentation in accordance with program guidelines 
Maintain HIPAA compliance and patient confidentiality 
Perform other related duties as mutually agreed upon with the Contractor 

Requirements

Qualifications 
Required: 
Current health coach certification 
Bilingual fluency in Spanish and English (native or near-native Spanish strongly preferred) 
Experience supporting adults with behavior change, lifestyle modification, or wellness 
Comfort working in virtual and community-based settings 
Preferred: 
National Board for Health & Wellness Coaching (NBHWC) certification 
Experience working alongside clinical teams or in healthcare-adjacent roles 
Familiarity with lifestyle medicine principles
